Ambercare® Home Health
Home Healthcare is for those individuals who may require assistance during an illness or after surgery. Services are provided in the comfort and privacy of one?s home to those persons who are in need of nursing, therapy treatment, social services and/or assistance with the activities of daily living.
Ambercare® Hospice
Comfort and Compassion When It?s Needed Most
Hospice is a special kind of care for people with a life-limiting or terminal illness. It is also a philosophy of care that allows the patient to direct their own care, provides for the management of symptoms, ensures comfort and dignity, as well as provides much needed help and support for the families and loved ones of the patient. The primary focus of hospice care is quality of life rather than aggressive treatment or cure of a disease process. Hospice care is provided by a dedicated team of professionals in the patient?s place of residence?their home, a nursing facility, residential care facility or assisted living facility.
Ambercare® Hospice offers bereavement services at no charge to the loved ones of patients who passed away while in our care as well as to the community at large. Our trained and compassionate staff is committed to walking with you through this most difficult journey and we encourage you to let us know how we can be of help to you.
Personal Care Option
The Medicaid Personal Care Option (PCO) is available to all Medicaid recipients who receive SSI, are over 21 years of age, and need assistance with at least two activities of daily living. Activities of daily living with which PCO clients may need assistance are:
Assistance with medications
Bowel and bladder services
Cognitive assistance
Eating
Household services
Hygiene / Grooming / Bathing
Meal preparation
Minor maintenance of assistive devices
Mobility assistance
Skin care
Support services
Disabled and Elderly Medicaid Waiver Program/Coordination of Long Term Care Services
D&E Waiver / COLTS
The purpose of The Disabled and Elderly Medicaid Waiver Program is to support elderly and/or disabled individuals in their homes as an alternative to institutionalization. Services provide support, which allow people with disabilities to live in their community among family and friends. Ambercare® Home Health provides Case Management, Homemaker, Homemaker Respite, and Nursing services to individuals eligible for the Disabled and Elderly Waiver program.
Private-Pay Home Health Services
Ambercare® Home Health can also provide individuals with private-pay services when patients are no longer eligible for traditional home care. Services may include: Nursing and personal care assistance. To assist individuals with bathing, dressing, grooming, light housekeeping and meal preparation.
